It's definitely his own fault but the Costa group must carry some responsibility for employing him as captain when he doesn't seem to have the experience. According to the BBC… "He attended a nautical institute in the nearby town of Piano di Sorrento and then joined Costa Cruises in 2002, initially as an official in charge of security. In 2006, he was promoted from second-in-command to captain."
