This crash appears to have been caused by gross mass stupidity, it wasn't an accident however you look at it.

Bus driver ignores red stop light and drives onto rail crossing.
Bus conks out.
Bus driver tries to get 55 passengers to leave the bus, many don't.
Train crashes into bus.
